Food Preservatives Linked to Increased Risk of High Blood Pressure and Cardiovascular Disease, Study Suggests

In an era dominated by processed foods, ingredients like potassium sorbate, citric acid, and L-ascorbic acid (vitamin C) are ubiquitous on supermarket shelves. These substances, commonly known as food additives or preservatives, are crucial for preventing spoilage and maintaining food quality. According to Open Food Facts, a leading open food database, over 20 percent of processed foods and beverages globally contain at least one preservative, highlighting their widespread integration into modern diets.
Against this backdrop, a significant research effort led by scientists from Sorbonne Paris Nord University and Université Paris Cité delved into the potential health implications of these additives. The team analyzed data from the extensive NutriNet-Santé cohort study, which meticulously tracked 112,395 participants for an average of 7.9 years. The primary objective was to investigate the relationship between dietary preservative intake and the risk of developing hypertension and cardiovascular disease, a largely unexplored area in human studies. Anaïs Hasenböhler, the doctoral researcher leading the study, noted, "Experimental studies suggest that some preservative food additives may be harmful to cardiovascular health, but we have not had enough evidence on the impact of these ingredients in humans. As far as we know, this is the first study of its kind to investigate the links between a wide range of preservatives and cardiovascular health."
The researchers categorized preservatives into two main types: non-antioxidant preservatives, such as sorbates, nitrites, and sulfites, which primarily inhibit microbial growth; and antioxidant preservatives, including ascorbic acid, citric acid, and erythorbates, which prevent oxidation and discoloration. The study revealed that nearly all participants, an astounding 99.5 percent, consumed at least one preservative during the initial two years of the study, underscoring the pervasive presence of these additives in our food supply.
The analysis yielded concerning findings: participants with the highest intake of non-antioxidant preservatives exhibited a 29 percent higher risk of developing hypertension compared to those with the lowest intake. Furthermore, this group faced a 16 percent higher risk of overall cardiovascular disease, encompassing serious conditions like heart attack, stroke, and angina. Similarly, those with the highest intake of antioxidant preservatives showed a 22 percent increased risk of hypertension. Delving deeper, the study identified eight specific preservatives linked to an elevated risk of hypertension, including potassium sorbate (E202), sodium nitrite (E250), ascorbic acid (E300), and citric acid (E330). Ascorbic acid was also independently associated with an increased risk of cardiovascular disease.
During the follow-up period, the study recorded 5,544 new cases of hypertension and 2,450 cases of cardiovascular disease. Intriguingly, approximately 16 percent of the association between non-antioxidant preservatives and cardiovascular disease was indirectly mediated through hypertension, suggesting that these additives might contribute to high blood pressure, which subsequently elevates cardiovascular risk. However, the researchers emphasized that this was an observational study, meaning it highlights associations rather than establishing direct causation. Important limitations include the disproportionate representation of women (78.7 percent) and a higher proportion of highly educated individuals within the cohort, potentially limiting generalizability to the broader population.
Despite these caveats, the statistical models meticulously accounted for numerous confounding factors, and the results consistently held across various sensitivity analyses. Mathilde Touvier, research director at the French National Institute of Health and Medical Research, called for action: "These results suggest we need a reevaluation of the risks and benefits of these food additives by the authorities in charge, such as the EFSA in Europe and the FDA in the USA, for better consumer protection." The findings underscore existing recommendations to prioritize non-processed and minimally processed foods, prompting a critical re-examination of current regulatory frameworks for additives consumed continuously without numerical limits.
